USAID Awards New Health Project in Western Kenya

Nairobi, Kenya — To improve the health of Kenyans in the western region of the country, the U.S. Agency for International Development (USAID) has awarded a comprehensive health service delivery project to the Program for Appropriate Technology in Health (PATH). The project, APHIAplus, builds upon USAID/Kenya's country-wide health service support project known as the AIDS, Population and Health Integrated Assistance II (APHIA II). PATH and its partners will implement the APHIAplus project in Western and Nyanza Provinces from January 1, 2011 to December 31, 2015 and work closely with the Government of Kenya and a wide variety of non-government agencies.

“Studies have shown us that diseases such as HIV, malaria, and tuberculosis are causing greater suffering and death in western Kenya than any other part of the country,” said Erna Kerst, the USAID/Kenya Mission Director. “This is why USAID is pleased to renew its commitment to this region through the APHIAplus project.” She further noted that in addition to addressing specific diseases, the APHIAplus project would work on strengthening the region's health care system by rehabilitating health facilities, training staff, and supporting supply and communication networks. Like APHIA II, APHIAplus will strengthen the country's ability to improve the lives of mothers, children and their families in the areas of HIV/AIDS, malaria, family planning, reproductive health, and tuberculosis with quality health services.

PATH's five-year award is valued at US$143 million and is one of several region-specific health projects that USAID/Kenya plans to award in the near future. The APHIAplus program is one part of USAID/Kenya's comprehensive health strategy that is valued at US$1.5 billion over five years. It is one of USAID's largest health investments and is an important part of President Barack Obama's Global Health Initiative – a 6-year, $63 billion support to international health.

USAID has been working in Kenya for more than 50 years. Today, USAID/Kenya is working with the Government of Kenya, civil society groups and the private sector to advance democracy and the rule of law, to improve health and education services, and to modernize and grow the economy so that Kenya's wealth is shared by more.

How can more information lead to transparency of International Development Aid?

For information to result in greater transparency and aid effectiveness it needs to be accessible and understandable. The USG needs to organize their information on AID spending into one easy to use site that comiles all spending by focus area.

Organizations Such as Aid Transparency are trying to encourage this.

Websites such as USA Spending and Fed Spending are steps in the right direction, but more work can be done to organize all aid activity by country and focus area as well as link in outcome data/effectiveness metrics to see if the money we are spending is actually making an impact in areas such as health outcomes and poverty.

Federal agencies are failing to maximize opportunities to make contracts competitive, often because of poor management or because officials have grown comfortable with incumbent contractors, according to a new report from the Government Accountability Office.

The watchdog reviewed trends in noncompetitive contracts during the past several years and discovered a number of questionable business practices by contracting officials and program managers. GAO found 44 percent of all federal contracts in fiscal 2009 either were not placed up for competition or attracted only one bid.

The report (GAO-10-833), which the House Oversight and Government Reform Committee requested, highlighted contracts that appeared to be written with such narrowly defined requirements that only one company could reasonably compete. In other instances, program offices pressed for follow-on contracts to be awarded without competition to the existing company because it would be more expeditious since the offices already had formed a relationship with the firm.

This type of legislation could be a signal of times to come. Cash strapped US may divert international development funding to Domestic programs.

$50 million in FY 2010 funding for the Millennium Challenge Corporation (MCC) was rescinded in order to provide funding for education and Medicaid as part of the amended bill H.R. 1586 (FAA Air Transportation Modernization and Safety Improvement Act; Public Law #111-226).

Review of Locally Employed Staff Compensation Issues

(a) Findings- Based on information obtained from the April 2009 report from the Office of the Inspector General of the Department of State and the Broadcasting Board of Governors, entitled ‘Review of Locally Employed Staff Compensation Issues’ (Report Number ISP-I-09-44), Congress makes the following findings:

(1) U.S. embassies and consulates worldwide retain over 51,000 locally employed (LE) staff under local compensation plans (LCPs) in about 170 overseas missions. A report by the Office of the Inspector General of the Department of State and the Broadcasting Board of Governors, entitled ‘Review of Locally Employed Staff Compensation Issues’ (Report Number ISP-I-09-44), stated that: ‘The U.S. is falling behind in providing a competitive compensation package for LE staff that is commensurate with their experience, technical skills, and responsibilities’.

On July 30, 2008, the Tom Lantos and Henry J. Hyde United States Global Leadership Against HIV/AIDS, Tuberculosis, and Malaria Reauthorization Act of 2008 was signed into law, authorizing up to $48 billion from 2009 to 2013 to combat global HIV/AIDS, tuberculosis, and malaria. This law authorized the U.S. Government, through the U.S. President's Emergency Plan for AIDS Relief (PEPFAR), to establish Partnership Frameworks with host countries to promote a more sustainable approach to combating HIV/AIDS, characterized by strengthened country capacity, ownership, and leadership.

Partnership Frameworks provide a 5-year joint strategic framework for cooperation between the U.S. Government, the partner government, and other partners to combat HIV/AIDS in the host country through service delivery, policy reform, and coordinated financial commitments.

USAID announces the first round of "GHI Plus" countries, as well as the program's governance structure

GHI activities are being implemented in the more than 80 countries where U.S. government global health dollars are already at work. Under GHI, the U.S. government will coordinate with partner country governments to ensure that investments align with national priorities and build capacity. Eight countries have been selected as the first set of "GHI Plus" countries. They are: Bangladesh, Ethiopia, Guatemala, Kenya, Malawi, Mali, Nepal, and Rwanda. These countries will receive additional technical and management resources to quickly implement GHI's approach, including integrated programs and investments across the spectrum of infectious diseases, maternal and child health, family planning, and health systems activities. GHI Plus countries will provide enhanced opportunities to build upon existing public health programs; improve program performance; and work in close collaboration with partner governments, across U.S. government agencies, and with global partners.

The purpose of this PD is to design and implement a health service delivery program that would respond to the Results as outlined in the USAID/Kenya Implementation Framework (2010-2015), whose strategic goal is “health outcomes and impact through sustainable country-led programs and partnerships.” USAID/Kenya’s new framework aims to balance ensuring health impact with strengthening the overall health system. This PD represents the service delivery component of the Implementation Framework, which builds on the successful interventions of the APHIA II program. Projects under this PD will provide critical service delivery services anchored at the district level in coordination with all national level technical assistance components envisioned in the Implementation Framework.

USAID/Kenya desires to ensure that its support to Kenyan service delivery programs is tailored to the specific needs and characteristics of distinct areas within the country. For this reason, USAID/Kenya foresees utilizing implementing agencies that will provide focused support within sub-national regions (hereafter referred to as “zones”), with approaches within each zone tailored to individual districts within provinces. The recipient will improve services at the provincial, district, facility and community levels. Because well-performing health systems are essential to provide quality health services, the recipient will be expected to collaborate and coordinate closely with implementing partners focusing on the systems strengthening components of the Implementation Framework.

USAID Contracting Reforms Slated for June, Shah Tells Congress

Rajiv Shah on Tuesday vowed to push ahead with controversial changes to the U.S. Agency for International Development’s procurement strategy following a review of the agency’s largest contracts. The goal is to strengthen in-house capacity and partnerships with actors in the field, the USAID administrator told Congress.

“I look and fully understand the need for change,” Shah told April 20 the Senate Appropriations Subcommittee on State, Foreign Operations and Related Programs. “The examples are everywhere.”

Leahy also prodded Shah on the timeline for the completion of the Global Acquisition and Assistance System, a software package USAID hopes will track more than $11.5 billion in acquisition and assistance transactions each year, according to a document on the agency’s website. The USAID administrator said GLAAS is scheduled for completion in June 2011.

MLI seeks to strengthen the leadership capacity of ministries of health in Ethiopia, Mali, Nepal, Senegal, and Sierra Leone to advance policy in three interrelated areas: health financing for equity, donor harmonization in health, and reproductive health. Key to MLI’s approach is the belief that stronger political leadership will improve health systems and ensure respect for the right to the highest attainable standard of health, particularly for the most vulnerable populations.

This Funding Opportunity Announcement (FOA) issued by the National Institute of Allergy and Infectious Diseases (NIAID), National Institutes of Health (NIH), solicits Research Project Grant (R01) applications to enhance the understanding of early events in mucosal transmission of HIV/SIV leading to the discovery of novel mechanism(s) of virus transmission, host cell-virus interaction and to advance new concepts for HIV prevention strategies/approaches. The emphasis of this FOA is discovery research focused on acquiring novel insights that may lead to the development of new conventional or outside-the-box approaches to vaccines and non-vaccine biomedical prevention strategies for optimizing mucosal protection against HIV/SIV. Investigators new to the HIV field and established HIV investigators are encouraged to apply.
NIAID intends to commit approximately $6.0M in FY2010 to fund 8-12 applications submitted in response to this FOA. Awards issued under this FOA are contingent upon the availability of funds and the submission of a sufficient number of meritorious applications.

Instructions on How to Find USAID max Salaries

The U.S. Office of Personnel Management publishes its revised ES-6 level compensation every year. For the new ES-6 maximum, please go to http://www.opm.gov/oca/10tables/pdf/es.pdf for this authoritative reference. In 2007, USAID’s Office of Acquisition and Assistance replaced its reference to the ES-6 designator with its own Contractor Salary Threshold (CST) in Automated Directives System (ADS) 302.3.6.10. At 302.3.6.10a, it states:

“The USAID CST is equivalent to the maximum rate for Federal agencies without a certified SES performance appraisal system, as published by the Office of Personnel Management at http://www.opm.gov/oca/. While the amount of the CST is derived from the SES system, there is no other connection between the USAID contractor salary policy and the Senior Executive Service system.”

Supporting the Scale Up of Comprehensive HIV/AIDS Prevention Services

The purpose of this program is to scale up a comprehensive and integrated package of HIV prevention interventions and services in selected districts in the republic of Uganda. Those districts include but are not limited to: Kalangala, Mubende, Mityana, Luwero, Nakaseke, Wakiso, Nakasongola, and Apac and are selected because they have a high HIV prevalence (>8%) and are currently implementing district-wide door to door HIV routine, confidential counseling and testing (HCT) programs through PEPFAR support. Door to door HCT is an excellent approach for accessing confidential HIV counseling and testing services to couples and families. It identifies many discordant couples, improves disclosure of sero-status, reduces stigma and therefore likely has a significant impact on HIV prevention. Post test clubs for HIV + clients and discordant couples have already been formed within the door to door programs in some districts.

If Chemonics and DAI can mobilize quickly and make a quick impact on the recovery effort than this side stepping of the competition rules is worth it, but we will have to track the efficiency in which these relief efforts get up and running and really make a difference as quickly as anticipated.
